IMMUNIZATION REQUIREMENTS FOR 2010-2011 SCHOOL YEAR.
Diphtheria, Tetanus, Pertussis (DTaP): 5 doses required.
4 doses acceptable if dose 4 given on or after the 4th birthday.
NEW: BOOSTER DOSE OF Tdap REQUIRED for 7th Grade if more than 2 years since last dose of Td.
Poliomyelitis (IPV/OPV): 4 doses required.
3 doses acceptable if dose 3 given on or after the 4th birthday.
No change.
Measles, Mumps, Rubella: 2 doses required. No change.
HEPATITIS B: 3 DOSES REQUIRED THROUGH GRADE 10*
VARICELLA (Chickenpox): 2 DOSES REQUIRED FOR KINDERGARTEN and 1st Grade; 1 DOSE REQUIRED GRADES 2-10* UNLESS HISTORY OF VARICELLA DISEASE DOCUMENTED BY A LICENSED PHYSICIAN. 2 DOSES RECOMMENDED FOR ALL AGES.
